The Community Matrons Team support housebound patients with multiple health and social care needs to prevent unnecessary hospital admission and provide better health outcomes.

This role isMon-Fri 9am – 5pm across Westminster.
  • Registered nurse or nurse associate
  • Current NMC registration
  • Experience of community health
  • Experience of monitoring patients with complex conditions and identify deterioration
  • Ability to undertake vital signs observations including urinalysis, weight, MUST scores
  • Venepuncture skills
  • Strong communication, administration and documentation skills
  • Time management skills
  • Knowledge and experience of using S1
  • Ability to work independently and remotely from colleagues
  • Ability to work across various locations in Westminster (this often requires walking with equipment in trolley bag)

This role will be working across the borough of Westminster. Supporting patients on the Community Matrons caseload with:

  • Monitoring of vital signs and medication compliance
  • Assisting in attending outpatient appointments (inc. escorting)
  • Liaison with other health and social care providers
