This exciting opportunity allows you to work across multiple settings and services, ensuring seamless, patient-centred care for some of our most vulnerable young patients.

In this role, you will develop, deliver, and evaluate individualised treatment plans in close collaboration with the child's named consultant. Your expertise will help establish clear pathways of care and facilitate effective communication through liaison and referrals to other agencies as needed.

Additionally, you will support compliance with national audit programmes, contributing to ongoing service improvement and quality standards.

If you are passionate about holistic, coordinated care and making a positive impact on children’s health journeys, this is your chance to be part of a dynamic team committed to excellence in paediatric healthcare.
