Job overview

Band 6 Team Leader- 37.5 hours

Our Complex Care Team is made up of:

The Children’s Continuing Care Team, which provides respite in the home and school setting for children with complex needs.

The Long Term Ventilation Team which provides packages of care in the home for children who require ventilation to assist their breathing.

Swanbourne Gardens which is a 4 bedded respite unit that provides overnight respite for children with complex needs.

As a team we strive to provide the children we support with a high standard of care.

Main duties of the job

As a Band 6 Team Leader working within the team you would work across the 3 teams based out of Swanbourne Gardens - our unique 4 bedded respite unit. Alongside this you will be required to assess, plan, implement and evaluate patient care in the child and families own home or in school / nursery.

Communication is an integral component of this role and communicating effectively and regularly with our teams and the parents and families is vital to ensure the effective management of our services and the provision of appropriate levels of support.

You will be at the forefront of teaching and assessing the competencies required for the staff to deliver day to day care for the children with complex health needs, this will include interventions such as care and management of tracheostomies, gastrostomies and ventilation support.

You will be expected to participate in the active management of the rota to ensure delivery of commissioned packages of care and the provision of supervision and appraisals for the all of teams and ensure all mandatory training is up to date.

As a Band 6 we would expect you to role model the trusts value and behaviour framework when working within the team.

Detailed job description and main responsibilities

To carry out the nursing process for children with complex health needs in the community, including assessment, planning and implementing patient care.

To deputise for the Band 7 Community Complex Care Manager.

To line manage our teams including the management of sickness and absence and the completion of appraisals and provision of support to our wider teams.

To ensure all training and competencies are up to date for all staff to maintain the safe delivery of care.

To complete and update the rota for staffing and children for the provision of commissioned continuing care and long term ventilation packages and respite care.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
