Successfully complete a locally approved orientation programme To support a clinical environment that is patient focused To assist the Heart Failure Cardiac Rehabilitation Programme attendees under the direction of the CNS ensuring they achieve physical, spiritual and psychological comfort To gather initial information, in paper format and via IT portals, about the programme attendees for screening by the CNS.
To assist with clinical care, record observations and undertake clinical duties that is allocated by the CNS Perform Venepuncture Perform 12 lead ECG and report to CNS for interpretation To assist the CNS in the management of the unwell patient and be an active member of the resuscitation team Following ABUHB policies and procedures to manage patient bodily fluids in a safe, supportive manner.

Using Making Every Contact Count (MECC) tools and techniques to have conversations with patients about how they might make positive improvements to their health and well-being. To provide pre-arranged educational information for the programme attendees in groups and/or 1:1 session.
To provide accurate verbal reports to the CNS and other members of the MDT in respect of the programme attendees that they have interacted with To communicate with programme attendees and their carers/family members with care and sensitivity, always maintaining confidentiality.
To be able to express sound reasoning for all aspects of clinical practice. To recognise own limitations within clinical practice/knowledge and seek advice from the CNS, to clarify the appropriate course of action in cases that are beyond their clinical experience To actively participate in clinical supervision on a regular basis To document all clinical activity accurately using agreed local and national standards.
To demonstrate an ability to communicate and work with service users and carers from diverse background. Technical/Administrative duties: To be responsible for filing and stock control as required. To participate in the collection of data for audit purposes.
To participate in general housekeeping activities within the department To ensure equipment used for assessment and treatment sessions is safe, clean, and available To ensure that there are appropriate forms and literature necessary for treatment sessions.
Self-development: To adhere to the Code of Conduct for Healthcare Support Workers in Wales, HCSW Career Framework and all other relevant Health Board policies and procedures To develop, and maintain up to date knowledge and skills appropriate to the grade and this post To recognise limitations and potential scope for development to enhance practice To have responsibility for identifying learning needs in consultation with the CNS.
To seek out opportunities to develop knowledge and skills both within, and outside of ABUHB To keep a portfolio of your achievements, accomplishments, and skills necessary for the role To review and reflect on your own practice and performance in line with ABUHBs guidelines To participate in regular professional supervision and annual appraisal (PADR) Service delivery and improvement: To contribute to the induction of students and new staff within the setting To undertake all procedures, always respecting the dignity, privacy, religious and cultural beliefs of service users To ensure enquires to the Cardiac Rehabilitation service are dealt with in an appropriate and professional manner To contribute to the smooth running of the programme and office base To participate in clinical governance and quality improvement as required by the service To investigate new treatment techniques or ways of working that may benefit the service To take part in audit activity as required by the service To undertake projects that will benefit the programme attendees This job description is not an exhaustive list of responsibilities but is an outline framework.
