An exciting opportunity has arisen for a post as a Band 6 District Nurse / Case Manager who can demonstrate a wide range of clinical and leadership skills to complement our District Nurse Out of Hours Team . You will be joining a hardworking and compassionate team, who work together to provide the best care for patients in our area.

Main duties of the job

In the role as a District Nurse / Case Manager you will work in partnership with our patients and their families to lead in the delivery of person centred, safe and effective care. The successful candidate will require the ability to demonstrate sound clinical knowledge and skills in the management of patients with complex needs, long term conditions and end of life care.

You will have extensive clinical experience and experience of case management, preferably within the community setting and be able to demonstrate recent and ongoing clinical professional development relevant to this post. You will be supported to develop your leadership, organisational and communication skills and how to manage challenging situations in a professional and flexible manner.

The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

We employ almost 5,000 members of staff, provide around 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health.

The results in the 2020 national NHS Staff Survey showed improvements in all elements of the safety culture theme, with 92% of respondents feeling their role made a difference to our patients.

Detailed job description and main responsibilities
  1. To act as an autonomous nurse taking on going responsibility for the care coordination of appropriate patients.
  2. Be responsible for the assessment, planning and implementation and evaluation of nursing care to individuals, groups of patients and their families on the district nursing caseload encompassing a rehabilitative and self-care approach delegating activities as appropriate.
  3. Promote clinical excellence and evidence based approach to care by complying with the agreed Trust policies and procedures maintaining own clinical competencies within a clinical leadership role.
  4. To act as the practice link with GP’s-in and Out of Hours and other general practice staff.
  5. Liaise with other Health Care and Multi-Agency professionals to provide a multi-disciplinary approach to care, accepting referrals for assessment as per referral criteria.
  6. Demonstrate, teach and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary.
  7. Ensure that equipment and aids, necessary for effective care, meet the needs of individual patients and are adequately maintained. Demonstrate and explain safe use of equipment for patient and/or carer where required.
  8. To ensure that own and community nursing team implement the End of Life (EOL) Strategy including adherence to all EOL tools, acting as a clinical expert in EOL care provision covering the 24 hour period.
  9. To ensure a safe working environment advising on all aspects of safety in the home.
  10. To be responsible for monitoring the provision of health care given to patients by all grades of staff within the District Nursing Team including following up assessment visits undertaken by other members of the team in a timely way.
