Community support worker - Coventry and Rugby Community Learning Disabilities

The Coventry and Warwickshire Partnership Trust provides comprehensive mental health and learning disability services to the population of Coventry, Warwickshire and Solihull. There are four Community Teams for adults with a Learning Disability throughout the organisation.

This post is based within the team that covers Coventry and Rugby.

You would be joining our community Learning Disability Nursing team There is strong multi-disciplinary working within the team and the contribution of Nursing services staff is highly valued. Successful applicants will receive excellent support and regular, good quality supervision and opportunities for continuing professional development.

Within the role, you will provide support to the nursing team and the wider CLDT in the form of assessment, intervention, and evaluation.

Provide holistic care for Adults with a Learning Disability and complex needsrequiring specific health interventions.

To be responsible for a caseload of service users who require close monitoring under the supervision of a Band 5 Community nurse.

Be allocated to work with service users and assist in the development of care plans and risk assessments along side the clinical team including undertaking observations and monitoring.

Complete relevant documentation and any mandatory charts relevant to the service users, highlighting any changes to the qualified staff.

Carry out all relevant aspects of care without direct supervision adhering to Coventry & Warwickshire Partnership Trust policy/procedure. Respond quickly and appropriately to constantly changing situations amidst frequent interruptions and distractions.

Must demonstrate a positive attitude when working alongside people with a learning disability with a commitment to helping them secure for themselves a valued and as enjoyable a life as possible.

Provide a rich and stimulating environment enhancing the lifestyle of the service users and their individuality.

Participate in team facilitation sessions which provide education, training and consultation to carers, families and providers of individuals with learning disabilities who are receiving a CLDT service

Provide therapeutic intervention in support of the wider multi-disciplinary team
