We are looking to secure a registered Band 6 Nurse to join our established Community Nursing Team to deliver excellent health care across St Helens.

This role is vital in supporting our aim to provide exceptional patient care.

Successful applicants must be:
  • Community experienced adult nurse and able to demonstrate extensive knowledge, analytical and the practical skills required
  • Able to demonstrate delivery of quality care, through assessment, planning, implementation and evaluation
  • An excellent communicator / listener, be non-judgmental and empathetic with patients, carers

An Enhanced DBS with barred lists for both Adults & Children is required for this post.
  • To provide skilled nursing care and advice to patients in their own homes and elsewhere e.g., residential homes to undertake a comprehensive specialist assessment of patients using research to underpin their practice.
  • To demonstrate the promotion of healthy lifestyle practices to the public by the utilisation of their teaching skills.
  • Manage clinical caseloads efficiently and effectively.
  • Prioritise health problems and intervene appropriately to assist the patient in complex and urgent situations.
  • To contribute to quality improvements via clinical governance, clinical supervision, and essence of care.
  • To respect confidential information obtained during your work and refrain from discussing such information without the consent of the patient / client or a person entitled to act on their behalf, except where disclosure is required by Law or by the order of a Court or is necessary in the public interest.
  • To liaise with other disciplines and members of the Primary Health Care Team in the provision of patient care.
  • Maintain clear, concise, legible, and contemporaneous patient records always, in line with Trust Policies and Procedures and Standards for record keeping NMC.
  • Under the Data Protection Act, staff using computers will be required to ensure that security procedures are followed.

Acute Care

Providing emergency and maternity services at Whiston, Southport and Ormskirk hospitals, and medical and surgical specialties across all our sites.

Primary Care

Providing primary care services at Marshalls Cross Medical Centre situated in St Helens Hospital.

Community Services

Providing adult community services for St Helens and a wheelchair service in Chorley, South Ribble, and West Lancashire. Our inpatient unit at Newton Community Hospital is where patients needing acute hospital beds can continue rehabilitation, freeing up space for more unwell patients.

We also provide urgent care at our Urgent Treatment Centre located in St Helens town centre.

Specialist Regional Services

We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.

CLINICAL & PROFESSIONAL RESPONSIBILITIES
  • Registered practitioners who are non-medical prescribers are accountable for their prescribing activities by ensuring they are aware of their Professional accountability and any restrictions or special considerations in their prescribing practice
  • Maintain and utilise advanced clinical skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable diagnosis, planning and treatment of the patient
  • To implement nursing care as per care plan as discussed with the Team Manager and to make accurate observations of the physical, mental, social conditions of the patient and / or carer
  • To implement care based on research and from any post registration course undertaken
ADMINISTRATIVE RESPONSIBILITIES
  • Ensure patient records are kept up to date and accurate and recording is done in line with Trust policies and procedures
TEACHING & TRAINING RESPONSIBILITIES
  • To ensure that their skills / knowledge is up to date and that continuing education and development is paramount to practice
  • Ensure that all mandatory training is accessed and kept up to date
  • To be aware of any changes in policy / procedure of the Trust
LINE MANAGEMENT/SUPERVISORY RESPONSIBILITIES
  • Delivering change within own sphere of practice when and were indicated to improve service
  • The post holder will be expected to prioritise own workload and ensure effective time management strategies are embedded in own practice and that of others
  • Effecting change within own sphere of practice when and were indicated
  • Making recommendations to improve services
  • Influencing, motivating, supporting, and enabling others to contribute towards the effectiveness and success of Community Health Services
  • To train, supervise and develop junior staff through clinical supervision and mentoring
RESEARCH & AUDIT
  • Qualified Nurses are required to evidence maintenance of their registration with the Nursing and Midwifery Council (NMC) and work to the NMC Code of Professional Conduct
  • To ensure nursing care is evidence based
  • To participate in clinical audits.
