An exciting opportunity has arisen for a full time Band 6 Diabetes Specialist Nurse to join the community Diabetes Service within Birmingham Community Healthcare NHS Trust Foundation (BCHC).
The successful candidate must be able to demonstrate an in-depth knowledge of diabetes nursing. Specialist knowledge will have been gained through previous substantial diabetes clinical experience, and the successful applicant will be able to demonstrate evidence of educating patients and health care staff alike.
Working clinically you will be supported by a team of Senior Diabetes Specialist Nurses and Consultants to develop enhanced diabetes, leadership and management skills.
The role will involve undertaking clinical assessments with hybrid working, virtually (with the use of digital platforms) and face to face in community clinics and patient’s homes.
You will be part of a progressive, enthusiastic and dynamic community diabetes specialist team and play an instrumental part in the advancement of the service.

The Diabetes Nurse Specialist will have responsibility in line with national and local guidelines and protocols to provide assessment of patients needs and advise on appropriate diabetes management. The role will involve empowering patients with the skills and knowledge necessary to attain independence in self-management.
Your role is to effectively manage a caseload of patients with diabetes utilising specialist clinical knowledge/practice.
To work alongside and build relationships with the local GP surgeries, secondary care diabetes colleagues and community inter-disciplinary teams, delivering high quality Diabetes Care across acute and community settings.
To provide structured education to both professionals and patients.
