We have a great opportunity for an experienced pharmacist to join our MHICS team in West Berkshire to support people suffering significant mental illness. The MHICS teams work in close collaboration with GPs / PCNs and community services to help people access appropriate services.

You will utilise your advanced clinical skills to improve the health of patients with significant mental illness in primary care, provide expert advice on complex mental health disease management, review patient medication to address polypharmacy and be an integral part of the multi-disciplinary team.

Candidates must have good organisational skills and ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families and carers.

The role is based across PCNs in Berkshire West.
  • To provide expert pharmaceutical advice on management of significant mental illness to GPs and support interventions to improve physical health where appropriate.
  • To lead on providing specialist mental health medicines information and medicines optimisation support to health care professionals working in primary care network to help achieve the objectives of supporting people with significant mental illness to receive optimal care with medicines in primary care
  • Carry out clinical audit and improvement projects for high-risk medicines such as lithium prescribing, compliance with the valproate ‘Prevent’ programme, benzodiazepine prescribing / deprescribing
  • To undertake medication reviews for primary care service users requiring specialist mental health input for appropriate pharmaceutical interventions including reviewing psychotropic medication initiated in secondary mental health care settings, physical health monitoring in line with national guidance
  • To work closely with relevant community mental health teams, inpatient specialist clinical pharmacists and MHICS pharmacists in other PCNs to ensure seamless pharmaceutical care is provided throughout each patient’s journey through the various services
