Responsibilities:
- To undertake visits daily, to service users' homes, within their caseload.
- To assess under biopsychosocial model
- Maintain timely interventions through holistic assessment for a caseload.
- To take accurate & detailed notes.
Qualifications and Requirements:
- Fully enhanced valid DBS
- Must have HCPC registration.
- Must have community experience
