Forensic Social Worker | Rotherham Doncaster and South Humber NHS Foundation Trust
You will practice in a recovery-focused manner and provide support focusing on principles of anti-oppressive practice, promoting choice, control, and independence, whilst ensuring robust safety plans are in place. You will be a champion for implementing human rights-based practice and safeguarding within the service, working with the MDT to encourage the Social Model of Mental Health is considered.
You will carry a caseload of service users with complex needs, within the learning disability low secure service and will actively address adversity and support these individuals due to high risk of social exclusion and discrimination, being solution focused through championing for equality.
Working in partnership with service users and carers, to support empowerment and social inclusion, collating feedback consistently to enhance services and care provision.
Gather data and information for assessment and risk analysis which may be complex, sensitive, and contentious and provide appropriate reports.Liaise with the multi-disciplinary team regarding assessment on analysis of risk in relation to public, staff and of re-offending and relapse.
Contribute to multi-disciplinary teams and participate with members to develop care plans in line with risk assessments and needs, considering the family and social context.
Assist in co-ordination of care in the context of different legislation with other professionals within services.
To produce reports to reflect needs of individual service user’s carers and issues of risk.
Engage with service users and families to ensure they are aware of their rights under the Mental Health Act, Mental Capacity Act and associated legislation.
Work in partnership with service users and carers to meet social care needs within a range of complex needsEnable service users and carers to exercise their rights and promote equality and diversity.
Link and maintain contact with statutory, voluntary, charity and external agencies to inform assessments and formulate care packages. Promote involvement of voluntary agencies to meet needs.
Manage work to ensure personal safety and security of self and others and contribute to development of safe working environment.
