CLCH Community Planned Care Services are looking to expending our community services and currently seeking motivated dynamic Band 5 Nurses to join our District Nursing service based in Brent that will give you many opportunities to flourish and further develop your skills and career.
We are currently looking at a 5-year plan to upskill our nurses to become aspiring leaders for the future, this includes prescribing.

If so, an exciting new opportunity is available for a Registered Nurse to join our planned cares Service in Brent. This will be a fixed term post for 12 months and there may be an opportunity for permanent after this period.
Brent Planned Care Services provide community nursing services and is split into five locality teams. This post will be located in one of the Brent localities. The post holder will work closely with the team leads/ clinicians to develop the service and maintain close working relationships with both acute and community services.
Main duties of the job
The post holder will undertake community nursing duties involving delivery of nursing services and holistic assessments of patients’ health needs using a person centred approach.
This includes providing quality and seamless nursing care to housebound patients and being proactive in co-ordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long term care institutions.
In addition, the post holder will be involved in facilitating self care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery
As a quality fellow the post holder will spend one half day per month working for their allocated shared governance council. Their role will be to support and encourage the council’s work in delivering the quality strategy.

Person specification
Qualifications
Essential criteria- Registered Nurse (RN)
- NMC Registration
- Has undertaken formal training in staff and team management
- Post registration experience in a NHS community environment
- Knowledge of recent government legislation and changes in community care delivery
- Evidence of other post-registration education and training
Experience
Essential criteria- Recent clinical nursing experience negotiating and liaising within a multi-disciplinary team
- Experience of providing holistic care that involves the multidisciplinary team.
- Supporting students and other learners e.g. HCAs teaching others in the practice
- Knowledge of general wound care
- Working with people with complex health needs
- Experience of undertaking general assessment by utilising evidence based care
- Previous experience in the Nursing process, care planning and fluent in record keeping
- Knowledge of recent government legislation and changes in community care delivery
- Understanding of resource management
- Knowledge of the audit process
- Ability to practice. critically analyse and evaluate research relevant to
- Post registration experience in a NHS community environment
Skills and Knowledge
Essential criteria- Knowledge of out of hospital care and the role of the Community Nurse
- Knowledge of how Equal Opportunities can be implemented in practice
- Knowledge of reasonable response to service user needs and prioritising and dealing with those needs with a required urgency.
