Job overview
Our Patient Flow and Discharge Co-ordinator will be a fundamental member of the Trustwide Site operational team to manage patient flow and discharge. They willmanage patient flow throughout the urgent and emergency care system in order to achieve the quality standards and to provide support to multidisciplinary teams in achieving this goal.
They will also oversee the trust discharge lounge operationally and manage patient flow for the elective pathway to provide a single point of contact on a named ward for patients, families, carers and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary
colleagues.
They will liaise effectively with professionals throughout the healthcare system in maintaining patient flow and ensuring live bed state information with existing PAS system, Swiftplus and nerve centre, manage community beds, ensuring fair and equitable allocation to community beds across Torbay and South Devon and ensure patients are repatriated in a timely manner as detailed in the Repatriation Policy
across the ICB and wider NHS Responsible for the management of private transport requests and management of the out of hours.
They will compile and report daily Trust performance updates and sitreps to NHS England/Improvement and commissioners.

Improving patient flow will ensure that patients are admitted in a timely manner to the most appropriate clinical area, leading to a reduction in patient transfers between wards.
The team aims to ensure that patients will be placed in an appropriate clinical setting where there is a competency-based skill mix, and optimum staffing levels to provide safe and consistent care. It is a highly demanding but very rewarding role, requiring multi-tasking, excellent communication skills along with a friendly approachable personality.
You will be joining a supportive small team lead by the Trustwide Site operations division leadership team. Based at Torbay Hospital, providing system wide management of patient flow and discharge.

Knowledge and experience
Essential criteria- • Proven relevant experience working in an acute Trust in an admin or clinical capacity to demonstrate understanding of hospital systems and processes relevant to capacity and safe patient flow
- • Ability to work with multiple IT/software applications, i.e. use of spreadsheets / compiling statistics
