Marie Curie, in partnership with University Hospitals Plymouth, is introducing a new service model aimed at enhancing end-of-life care, with a special focus on identifying patients who are approaching the end of their lives. This innovative service focuses on four key areas: Delivering hands-on care and emotional support to patients and their families in the Emergency Department.
Proactively identifying and assisting those nearing the end of life within the acute hospital, ensuring timely transfers to their preferred place of care. Offering specialised care within Mount Gould Community Hospital. Engaging a dedicated team of volunteers to provide companionship and advance care planning.
This is an exciting time to be joining us as a Registered Nurse, where you will play a crucial role in achieving these outcomes and making a meaningful difference in the lives of patients and their families. In this role, you will work closely with teams and patients loved ones to provide comprehensive, person-centred care for those nearing the end of life.
This includes conducting holistic assessments, developing advance care and treatment escalation plans, and ensuring high-quality, evidence-based care is implemented, evaluated, and tailored to the needs of both patients and those important to them.
