Outreach Family Support Worker

Are you passionate about making a difference in the lives of children, young people, and families? Service Care Solutions are seeking a dedicated Outreach Family Support Worker to provide high-quality, home-based early help support using a 'whole family' approach.
You will play a crucial role in preventing the need for statutory services while ensuring families have access to universal support.
- £17.00 LTD per hour inclusive of holiday pay
- 35 hours per week
- 2 month initial contract with possibility of extension after this
- Hold and manage complex cases stepping down from children's social care (Level 2/3), completing early help assessments and coordinating Team Around the Family (TAF) meetings.
- Provide intensive support to children, young people, and families, including adolescents with complex emotional and mental health challenges.
- Assess risk and need using early help assessments, taking on the Lead Professional role where required.
- Develop and coordinate family support plans to reduce risk and improve outcomes.
- Support families in developing parenting strategies, promoting positive relationships.
- Maintain accurate records using the electronic case management system.
- Conduct home visits and provide outreach support.
- Deliver supervised contact sessions for families under Early Help intervention.
- Promote safeguarding in accordance with statutory guidance and legislation.
- Collaborate with multi-agency teams, including schools, social workers, and healthcare professionals.
Requirements
Essential:
- NVQ Level 3 in Childcare, Education, or Health & Social Care.
- Experience of direct work with children, young people, and families.
- Strong skills in case management, assessment, and delivering interventions.
- Effective communication skills, both written and verbal.
- Experience in providing home-based and outreach support.
- Understanding of safeguarding and child protection procedures.
- Ability to work independently and within a team.
Desirable:
- Experience in parenting support and family mediation.
- Knowledge of child development and emotional well-being.
- Experience of working with families under SEND guidelines (ages 19-25).
- Strong IT skills and the ability to maintain accurate case records.
