Outreach Family Support Worker

Are you passionate about making a difference in the lives of children, young people, and families? Service Care Solutions are seeking a dedicated Outreach Family Support Worker to provide high-quality, home-based early help support using a 'whole family' approach.

You will play a crucial role in preventing the need for statutory services while ensuring families have access to universal support.

  • £17.00 LTD per hour inclusive of holiday pay
  • 35 hours per week
  • 2 month initial contract with possibility of extension after this
Responsibilities
  • Hold and manage complex cases stepping down from children's social care (Level 2/3), completing early help assessments and coordinating Team Around the Family (TAF) meetings.
  • Provide intensive support to children, young people, and families, including adolescents with complex emotional and mental health challenges.
  • Assess risk and need using early help assessments, taking on the Lead Professional role where required.
  • Develop and coordinate family support plans to reduce risk and improve outcomes.
  • Support families in developing parenting strategies, promoting positive relationships.
  • Maintain accurate records using the electronic case management system.
  • Conduct home visits and provide outreach support.
  • Deliver supervised contact sessions for families under Early Help intervention.
  • Promote safeguarding in accordance with statutory guidance and legislation.
  • Collaborate with multi-agency teams, including schools, social workers, and healthcare professionals.

Requirements

Essential:

  • NVQ Level 3 in Childcare, Education, or Health & Social Care.
  • Experience of direct work with children, young people, and families.
  • Strong skills in case management, assessment, and delivering interventions.
  • Effective communication skills, both written and verbal.
  • Experience in providing home-based and outreach support.
  • Understanding of safeguarding and child protection procedures.
  • Ability to work independently and within a team.

Desirable:

  • Experience in parenting support and family mediation.
  • Knowledge of child development and emotional well-being.
  • Experience of working with families under SEND guidelines (ages 19-25).
  • Strong IT skills and the ability to maintain accurate case records.
