Physiotherapist

We are seeking to recruit a highly motivated, experienced and enthusiastic physiotherapist to join the very busy Community Learning Disability Team.

You will be working as part of a small team of physiotherapists and physiotherapy assistant practitioners within many different community settings in the south district of Midland’s Partnership Foundation Trust.

The post holder would also work as part of a wider multi-professional team consisting of Nurse, Occupational Therapists, Psychiatrists, Psychologists and Speech and Language Therapists. They would also work collaboratively with mainstream services, statutory and Independent sector services.

The post holder will work with people with complex healthcare needs within their own homes or residential care homes, day services and other community and hospital settings.

The ideal candidate will be somebody who is passionate about providing high quality service to people with learning disabilities in community settings and someone who can work creatively to secure good outcomes, embracing the values shared by this friendly and supportive team.

The work will be primarily adult clients with learning disabilities and physical disabilities.

Main duties of the job

JOB PURPOSE:

To provide a highly specialist Physiotherapy Service to adults with complex learning and physical disabilities; undertaking clinical diagnosis, assessment and physiotherapeutic management and interventions.
To identify and manage those with high risks promptly and pro-actively.

To deliver a high standard of care using an holistic, person-centered approach on a daily basis and providing clinical leadership, teaching and supervision to members of the multi-disciplinary team.

You will be expected to be able to prioritise work and meet deadlines in order to fulfil the requirements of the post. Excellent interpersonal and communication skills are essential with an ability to deal with highly sensitive and challenging situations, communicating to both internal and external service users.

Detailed job description and main responsibilities

KEY DUTIES AND RESPONSIBILITIES:
  1. Carry out comprehensive, evidence-based physiotherapy assessments for clients with complex needs. Interpret clinical and non-clinical information using clinical reasoning skills to accurately diagnose and develop specific treatment plans. Provide intervention and/or recommendations for appropriate intervention for a small complex caseload. Interventions will include physiotherapeutic handling, postural management, respiratory support via blended working and some face to face clinics
  2. Use high level clinical reasoning skills to identify, evaluate and prioritise clinical need.
  3. Provide physiotherapy assessments and expert advice to other professionals, clients, carers and family to meet clients individual care needs in relation to physiotherapy treatments / interventions.
  4. Ensure that clients and their families and/or carers are involved in all aspects of physiotherapy input. Negotiate with clients and carers to identify and formulate client-centred goals and demonstrate ongoing evaluation of client progress using agreed outcome measures.
  5. Ensure that equipment identified during assessment is ordered and provided as required including seating, positioning and mobility equipment, orthotics and wheelchairs. Ensure that items of equipment are safe to use and that those using the equipment are competent to do so.
  6. Work collaboratively with the multi-disciplinary team, clients, advocates, paid carers and family carers to provide a model of intervention that addresses the client’s individual needs.
  7. Supervise and provide specialist physiotherapy advice to other members of the multi-disciplinary team in relation to complex cases.
  8. Triage and prioritise urgent, unexpected and planned referrals, adjusting caseloads and allocations to meet revised needs.
  9. Gain and document consent or Best Interest decisions for physiotherapy interventions.
  10. Actively participate in and chair when required Multi-disciplinary team meetings, including Professionals Meetings, Care Programme Approach, Best Interest meetings and safeguarding.
  11. Write detailed reports and programmes on client’s progress and rehabilitation in line with identified needs.
  12. Develop outcome measures for and with service users and their carers. Monitor service user’s progress as necessary and discharge them from the service as appropriate.
  13. Design, develop and provide training in physiotherapy interventions to paid carers, families and other communication partners, service users, other professionals within and outside the immediate multidisciplinary team.
  14. Promote physical activity and healthy lifestyles to prevent secondary complications.
  15. Identify, assess and manage clinical and other risks. Record near misses and actual events through the incident reporting and safeguarding processes.
  16. Actively participate in the transition process to other services and / or referral to other professionals or agencies as required.
  17. Delegate programmes and work to Assistant Practitioners.
  18. Work closely with other physiotherapists in the wider Adult Learning Disabilities team to develop the service and provide seamless transition between services.
  19. Carry out audits to improve the service and compliance with local and national standards.
  20. Add to the evidence base related to this specialist area of physiotherapy. Actively participate in initiatives aimed at improving physiotherapy services for people with a learning disability…

Qualifications

Essential criteria
  • Qualified Physiotherapist, registered with HCPC and CSP.
  • Evidence of post qualification courses and experiential learning related to the needs of adults with learning disabilities and physical health.
  • Specialist training or experience in learning disability.

Experience

Essential criteria
  • Autonomous assessment and management of complex physical problems of adults with learning disabilities and possibly challenging behaviour.
  • Risk assessment, clinical reasoning and risk management of this client group.
  • Working in a community multidisciplinary team.
  • Working in a range of settings including community residential, day services and domiciliary settings.
Desirable criteria
  • Experience of work in a neurology setting.
  • Member of ACPPLD.

Skills and knowledge

Essential criteria
  • Up to date and relevant knowledge of evidence based practice, e.g. NICE Guidance.
  • Knowledge of regulatory bodies and the requirements associated with this in terms of standards of service delivery, e.g. CQC.
  • IT skills to record patient and organisational information and to write reports and advise on reasonable adjustments and especially accessible materials.
  • Ability to lead effective and sustainable change.
