Key Responsibilities:
- Deliver comprehensive clinical pharmacy services across a network of GP practices.
- Collaborate with healthcare professionals to optimize medication use and improve patient outcomes.
- Conduct medication reviews, provide prescribing guidance, and manage complex medication regimens.
- Contribute to the development and implementation of clinical pathways and protocols.
- Participate in audits, clinical governance activities, and service improvement initiatives.
Essential Requirements:
- GPhC registration
- Minimum 2 years of clinical pharmacy experience within a primary care setting.
- Excellent communication, interpersonal, and organizational skills.
- A strong commitment to patient-centered care and improving healthcare services.
