Community Physiotherapist - Band 6
Utilising specialist knowledge and clinical reasoning, to plan and provide clinical interventions in consultation with other colleagues in the multidisciplinary team and to work in a range of settings as appropriate (e.g. patients home, short term placement or other community setting) To adopt a patient centred and sensitive approach to therapy, collaborating with patients (and/or carer(s) where appropriate) in all aspects of the therapy process (assessment, planning, intervention, evaluation, reporting).
To utilise individual and/or group work skills as appropriate in order to achieve agreed therapeutic goals To have a high level of knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls and specialist knowledge of at least one of these clinical areas To work collaboratively with Social Services and third sector providers to ensure delivery of effective safe care.To complete holistic assessments of patient needs with a focus on enablement, home first and effective utilisation of resources To take a lead for information management to ensure high quality data which is recorded, evaluated & reported in a timely manner.
To work collaboratively with the team leaders and LIT manager to participate in evaluation of the OCIT service and identify opportunities for improvement and service developmentFor further details of the role please see the attached job description.
