Practice Nurse/Nursing Associate
To provide within competence framework, clinical assessment, diagnosis, treatment and evaluation of care for the patients. They will also participate in chronic disease management clinics, immunisation, cervical cytology, health promotion and the day to day duties carried out by the nursing team.
They will work closely with the practice team to meet the needs of the patients in line with Practice Objectives and National Guidelines. Contributes to the overall Practice targets and objectives. Core Behaviours/Knowledge & Skills. Must have undergone and completed fully recognised Nurse Training.
Have previous experience working in a primary care setting. Required to work within the Code of Professional Conduct and other directives as defined by the NMC. Excellent communication skills both written and verbal are essential. It is desirable that the applicant has some basic computer skills with some experience of clinical systems for example System One, The applicant must be able to work as part of a team, have the ability to organise and prioritise own clinical caseload on a day to day basis.
Responding effectively to patient need and ensuring patient choice and ease of access to services. Training and Personal Development The post holder will be required to take responsibility in agreement with the Practice Manager, Nurse Practitioner and General Practitioners for his/her own personal development by ensuring that Continuous Professional Development remains a priority.
To maintain their professional registration. Attend relevant courses and study relevant to the post. To work flexibly as the different primary care nursing specialities develop. Ensure they attend relevant meetings both internally and externally.
Training needs will be agreed and supported by an annual appraisal. The post holder will undertake all mandatory training required for the role. Time will be allocated within paid practice hours for mandatory training and any other training that is requested by the GPs to fulfil the role.

Undertake necessary invasive and non- invasive diagnostic tests or investigations and interpret findings/reports within the competency framework Provide safe, evidence based, cost effective, individualised patient care Offer a holistic service to patients and their families, developing where appropriate an on going plan of care/support with an emphasis on prevention and self care Promote health and well being through the use of health promotion, health education, screening and therapeutic communication skills To be actively involved in achieving practice targets.

Specific areas of responsibility will be set by the General Practitioners in line with knowledge and experience of the individual To keep accurate and immediate records of every patient contact using the software system provided Organise various in house clinics and help manage call/recall programmes in line with patient needs and practice protocol.Comply with Adult and Child Protection policies Comply with all other practice policies and procedures, paying particular attention to COSHH, Health & Safety, Infection Control, Manual Handling and Confidentiality Agree to provide any other clinical duties as deemed necessary by the General Practitioners Main Clinical duties Blood pressure monitoring and as part of new patient checks.
Reporting any abnormalities. Phlebotomy/ Venepuncture. Following health and safety policies regarding safe handling/disposal of sharps/needle stick policy Prepare samples for laboratory. Urinalysis. Report any abnormalities to the clinical team.
