Job overview

An exciting opportunity has arisen for an experienced Qualified Band 6 Mental health practitioner to join The Fylde Frequent Attenders Team. This post will cover Blackpool, Wyre and Fylde localities.

Service users within the “Frequent Attender Programme” are high intensity/complex individuals with multiple issues and needs. They can be frequent attenders within the Emergency Department or come to the attention of emergency services such as the police and be subject to multiple Sc 136s.

They may also frequently utilise other emergency services such as the police or NWAS. Individuals may will also be receiving care from Home Based Treatment Team or CMHT.

Main duties of the job

We need staff who are highly motivated and proactive and who are able to work assertively and empathically with people within the locality to encourage their engagement with multi agency services and support them within their recovery. This will involve working collaboratively with other agencies to aid engagement to meet the needs of the service user.

The B6 MHP’s will provide a comprehensive assessment and develop collaborative care plans with Service Users and other agencies and where necessary signpost to other services within the community. For those Service Users that are already known to mental health services you will work collaboratively with the Key Worker to develop a robust plan of care (or dovetail care plans) that Multi agencies are involved with to support the person and prevent crisis.

Education

Essential criteria
  • Registered Mental Health Practitioner
  • Social Work equivalent qualification
Desirable criteria
  • Practice teacher qualification/student supervisor

Knowledge

Essential criteria
  • Able to understand the difference between crisis planning and contingency planning and able to formulate explicit planning in these areas
  • Knowledge, understanding and experience of the care pathways for psychosis and non psychosis and relevant interventions
  • Effective communication with service users, carers and members of the multidisciplinary teams. Strong customer service and negotiation skills

Skilla and Abilities

  • Motivational skills
  • Assertiveness skills in relation to bed management
  • Demonstrate the ability to work sensitively in a non judgemental way with service users who are in crisis
