Main duties of the job
  1. To lead and work alongside the clinical pharmacy technical team to:
  1. Optimise medicine management by taking drug histories, assessing patients’ own drugs at admission for use during their hospital stay and at discharge and promoting one stop dispensing
  2. Resolve and/or appropriately refer interventions back to the clinical pharmacist or liaise with other members of the MDT within scope and experience.
  3. Record details of medication history, patient’ own supply and any other relevant information on the electronic in-patient prescription chart in accordance with pharmacy procedures
  4. Undertake Level 2 medicines reconciliation within competency in line with appropriate policies and procedures.
  5. Document interventions and discrepancies related to prescribing of medicines on the electronic patient records.
  6. Encourage and support compliance with the local formulary and prescribing policies and guidelines.
  7. Effectively communicate with patients regarding medicines supply and to identify administration or adherence problems.

The post holder is required to:

  • Work within agreed Trust and departmental policies and procedures and to accepted standards of practice.
  • Work in other areas and on other sites in the Trust to cover for service needs.
  • Undertake such other duties as may be required from time to time, as are consistent with the responsibilities of the grade.
  • Ensure confidentiality at all times, only releasing confidential information obtained during the course of employment to those acting in an official capacity.
  • Comply with the Code of Ethics of the Royal Pharmaceutical Society of Great Britain.
  • Treat patients and customers with dignity and courtesy.
  • Ensure that medicines do not fall into unauthorised hands.
  • Have responsibility for the health, safety and welfare for self and others and to comply with the requirements of the Health and Safety regulations, including COSHH.
  • Promote Equal Opportunities and to comply with the requirements of the legislation and the Trust’s policies.
  • Attend such training courses and meetings as are necessary for the efficient performance of the post holder’s duties.
  • Participate in Continuing Professional Development, maintaining own CPD portfolio.

QUALIFICATIONS / EDUCATION

Essential criteria
  • Masters in Pharmacy Degree – MPharm 1 year pre-registration training Professional registration with GPhC* Diploma in Clinical Pharmacy or equivalent
Desirable criteria
  • Management qualification or training Membership of Royal Pharmaceutical Society of Great Britain

Experience

Essential criteria
  • Significant post registration hospital experience Experience in managing a defined service Service Development Staff training and appraisal Staff management Experience of clinical trials Demonstrates evidence of excellent clinical knowledge and skills Principles of Clinical Governance Risk assessment
Desirable criteria
  • Resource management Experience using KPIs to monitor a service Complaint handling Staff recruitment and retention Knowledge of CMM pharmacy computer system
