Are you ready to elevate your career and make a real impact? Join us in the vital role of coordinating a home care service for patients battling chronic obstructive pulmonary disease (COPD) and ensuring ongoing support for this essential client group!

In this dynamic position, you will take the helm, responsible for determining clinical diagnoses and crafting individualized treatment programs that truly meet patients’ needs. Your expertise will shine as you collaborate with the Home Oxygen Service to assess and facilitate the installation of home oxygen, ensuring every patient receives the care they deserve.

You’ll be the driving force behind seamless pathways of care, liaising and referring to other agencies as needed, ensuring comprehensive support for all respiratory conditions outlined in our service contract. Work alongside our Pulmonary Rehabilitation Lead to innovate and enhance our services, boosting accessibility and capacity while maintaining the highest standards and outcomes.

Your commitment to excellence will mean ensuring compliance with professional policies and guidelines, while also facilitating timely reviews of admissions and referrals from Emergency Admissions Units and medical wards. You’ll ensure every patient is discharged home with the right treatment package, and you’ll conduct home visits to deliver care tailored to their clinical needs.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months.

Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.
