To work in close collaboration with other professionals in an Assessment & Treatment environment in assessing patients needs, devising Person Centered care plans and evaluating outcomes that promote patient empowerment through the process. You with the wider Multi disciplinary Team will be expected to participate in the Assessment & Treatment to provide high quality, individualised care, which addresses the needs and wants of adolescents and adults with learning disabilities and/oroverlying psychiatric disorders or challenging behaviours.
