Our service delivers highly specialist diabetes care to patients with Type 1 diabetes, complex Type 2 diabetes and provides in
  • patient care.

We cover the diabetes centre and inpatients across the LTHT sites.

We are looking for a diabetes specialist nurse who is enthusiastic and committed to the provision of excellence in diabetes care and wants to further develop their knowledge and skills in this field.
The successful applicant will strengthen our diabetes specialist nursing service, providing expert advice to patients and education to staff.

Training will be given to provide expert care in highly specialist clinics to include; antenatal, preconception, renal, foot, young adult, diabetes technology and DAFNE.
The jobholder carries responsibility for the assessment of care needs, the development, implementation, and evaluation of programmes of care.
The jobholder will provide specialist nursing care and advice to a specific patient group and will contribute to the development of the speciality service.
The jobholder will ensure cost effective use of resources.
Our service provides a 7 day a week diabetes specialist nursing cover across LTHT.
The jobholder would be expected to work flexibly with colleagues to provide this cover on a rotational basis.
  1. JOB DIMENSIONS The current nursing service comprises: Band 8 Lead Nurse for Diabetes Band 7 Senior Sister for Diabetes Band 6 Diabetes Nurse Specialists Clinical Support workers The post holder will work within a team of Nurse Specialists and allied health professionals.

KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED Qualifications Registered Nurse (Level 1) Recognised teaching/assessing qualification Post registration qualification in specialty Experience Significant experience working as a Band 5 Staff Nurse or equivalent post registration experience of which there has been substantial experience gained within the relevant specialty area, during which time the following experience should have been gained: Clinical audit and standard setting Service development and or change management (desirable) Skills Teaching Communication Leadership IT Skills Knowledge Experience and knowledge of speciality Current issues in healthcare Standards of professional practice Personal Attributes Excellent interpersonal skills Innovator Motivator Assertiveness Flexibility, adaptable, capable of lateral thinking 6.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY Acts in accordance with the Nursing and Midwifery Code of Conduct Clinical care delivery
  • Provide a clinical lead and role model to nursing staff by delivering high quality care.

  • Provide education and training to patients and their carers.

  • Investigate and respond to accidents, complaints, untoward incidents and other significant events
  • Maintains record of workload and performance monitoring information and reviews this on a regular basis with team manager.
  • Work with Team Leader and/or Matron to promote the development of nurses undertaking the CNO Ten Key Roles as considered appropriate
  • Under the direction of the Team Manager participates in or leads the development of specific elements of nursing practice, reflecting the CMT and Nursing Directorate business plans
  • Participates in the setting, monitoring and improvement of standards and quality of patient care, including those standards defined in the National Benchmarks for Essential Care
  • Promote evidence-based practice, wherever possible
  • Engage users and carers views on service delivery and development.
  • Implement policy and practice guidelines specific to the clinical specialty, proposing changes where appropriate
  • Contribute to policy and protocol development for speciality area.
  • Responsible for the day to day supervision of junior clinical staff and students within the specialist team.
  • Exercises a personal duty of care in relation to equipment, resource and stock control utilising equipment and supplies appropriately.
Research and Audit
  • Contribute to the research agenda
  • Undertake audit programmes specific to client group, treatments and developments.
  • Participate in clinical trials as appropriate.

Health and Safety/Risk Management All staff are responsible for working with their colleagues to maintain and improve the quality of services provided to our patients and other service users.

This includes complying at all times with the Leeds Teaching Hospitals NHS Trust Policies, including Health and Safety policies, in particular by following agreed safe working procedures, and reporting incidents using the Trust Incident Reporting system.

Infection Control The jobholder must comply at all times with the Leeds Teaching Hospitals NHS Trust Infection Control policies, in particular by practising Universal Infection Control Precautions.
Hand hygiene must be performed before and after contact with patients and their environment.
