Discharge Flow Facilitator
Inputting information onto active clinical notes. Supporting the wards with paperwork required to discharge a patient Assist in the process of patient flow through to ensure compliance with the Trust KPIs Build strong communication skills with all members of the Multi Disciplinary Team ensuring that the most up to date patient information is available Address discrimination and harassment, and promote equality and diversity in the workplace Reduce sickness absence work place accidents and promote zero tolerance on violence against staff Establish good working relationships throughout the hospital and with partners in the Local Authorities.
Deal with queries from service users including patients and their representatives in a manner that is consistent with legislation, policies and procedures whilst demonstrating understanding, empathy and discretion, taking accurate written messages and passing more complex queries to relevant staff members where appropriate.Assist in planning and prioritising of workload to ensure that all service requirements and priorities are met Data entry and retrieval of results/information where appropriate through the effective use of available technologies Under the direction of the Senior Clinical Professional/Site Managers, assist in providing interim short-term cover throughout the work area where appropriate to cover sickness absence/annual leave where appropriate Assist in ensuring a healthy, safe and secure working environment for self and work team Any other ad-hoc duties as and when required in line with service needs and within scope of capabilities Demonstrate commitment to IWL principles and flexible working patterns, to meet the needs of the service and staff Take responsibility for own education/training Supporting enhanced care caseload when applicable
