Department Summary:

HMR Older People’s CMHT is the community mental health team for older people. It is a borough wide team and provides a service to Rochdale, Middleton and Heywood. The team is comprised of community mental health nurses, an occupational therapist, team manager and an administrator.

Role Summary:

To undertake the role of care co-ordinator in the assessment, treatment, and delivery of care to older people with mental health problems, who meet the criteria for access to the service.

You will ensure that the highest quality care is carried out in accordance with the policies and procedures of Pennine Care NHS Foundation Trust and the Nursing and Midwifery Council.

Main duties of the job

To undertake holistic mental health and risk assessments using trust approved documentation and procedures.

To manage a caseload of clients who will have a variety of mental health conditions, both organic and functional.

To provide evidence based therapeutic interventions promoting enablement, choice, and recovery.

To comply with all legislative requirements in relation to service users care.

The post holder will support the duty system that is operated within the team and respond to crisis calls they receive.

Actively participate in multi-disciplinary meetings and contribute to clinical decision making.

Engage and supervise student nurses.

This role does not involve commissioning services on behalf of the local authority.

Education / Qualifications

Essential criteria
  • Professional qualification Mental Health Nursing - Registration with NMC.
  • Willingness to undertake further training relevant to the role resulting from legislative or service changes, as appropriate.
Desirable criteria
  • Evidence of post-basic qualification and training
  • MSLAP or equivalent Mentor training

Experience

Essential criteria
  • Experience in community mental health work
  • Experience in working with people with moderate to severe mental health needs
  • Experience of supervision
  • Experience of MDT working
Desirable criteria
  • Experience of leading a team / ’acting up’
  • Experience of managing or participating in change

Knowledge

Essential criteria
  • Knowledge of mental health legislation
  • Knowledge of CPA policy
  • Extensive knowledge / experience in social inclusion and recovery model of care.

Skills and Abilities

Essential criteria
  • Ability to care coordinate and be responsible for a caseload
  • Ability to work effectively as part of a MDT
  • Ability to supervise junior staff / students

  • Excellent written and verbal communication skills. Recording relevant documentation to a professional standard.

Work Related Circumstances

Essential criteria
  • Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs.
  • Willing to carry out all duties and responsibilities of the post in accordance with the Trust’s Equal Opportunities and Equality and Diversity policies.
  • Appointments to regulated and controlled activities require an enhanced DBS disclosure.
