The Triage command Centre based out of Moorlands View at Lynfield Mount Hospital is a service which has been recently set up to triage and manage referrals into the District Nursing Service. The team has a small cohort of staff and has further support from members of the planned and unplanned district nursing service .What we need from this role is some body that has great leadership skills , is solution focused, has some operational skills to manage Rota's, staff supervision and support. A back ground in community nursing would be an advantage , however it is essential that you have experience of managing at team leader level and have a good level of IT skills .
The team currently operates Monday - Friday 0800-1600, however, this is subject to change and may include weekends and bank holidays .

Main duties of the job
The post holder will provide leadership, and take managerial and supervisory responsibility to a group of practitioners working as a multidisciplinary team providing adult physical health services [APH] in the community.
The Team leader will provide leadership and coordination of BDCT adult physical health services to a defined Community area ensuring service delivery and responding to the needs of the Community as it develops as an Accountable care provider.Key partnerships will include GPs, mental health services, learning disability services, social care, Voluntary Sector providers. It is anticipated the roll will be 80% operational leadership and 20% clinical leadership

Qualifications
Essential criteria- District Nurse Qualification SPQ or equivalant professional qualification
- Must have current or recent operational management experience
- Evidence of further management study and/ or qualification
Experience
Essential criteria- Significant experience working as a band 6 DN Caseload holder/Team Leader or equivalent profession
- Experience of managing and developing staff
- Experience in both health & social care
