The Advanced Nurse Practitioner (ANP) will deliver a high standard of patient care, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base. Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
Will independently prescribe appropriate medication, evaluate, or refer to other specialists if necessary Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice. Support On call Clinician Implement and evaluate individual specialised treatment plans for chronic disease patients.
Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
Undertake the collection of pathological specimens. Request pathology / radiology services as necessary. Process and interpret pathology and other test results as required. Sign electronic prescriptions. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
Maintain accurate clinical records in conjunction with extant legislation. Ensure SNOMED CT codes are used effectively. Carry our home visits Prioritise health issues and intervene appropriately. Support the team in dealing with clinical emergencies.
Recognise, assess, and refer patients presenting with mental health needs. Be an extended and supplementary prescriber. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required.
Contribute to practice targets (QOF, etc.), complying with local and regional guidance. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.) Take part in audit work Support the clinical team with all safeguarding matters in accordance with local and national policies.
Deliver opportunistic health promotion where appropriate. Help deliver national immunisation campaigns. Attending local training events Attend and participate in practice meetings Professional Role To be a lead for practice clinical areas of skills and interest.
To promote personal development and clinical excellence. Working with the nursing team and others in developing new roles, responding to changing healthcare needs. To facilitate and participate in multi-disciplinary education and clinical supervision.
Participate in organisational decision making, interpret variations in outcomes and use data from information systems to improve practice. To maintain own professional development in line with the Nursing and Midwifery Council Regulations. To promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.

Monitor the effectiveness of their clinical practice through the quality assurance strategies such as the use of audit and peer review Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained To play an active role in the development of practice development plans and new services.
