Youll apply your gained communication and relationship building skills to effectively manage rotas for your care teams, respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families and other healthcare professionals.
No matter which care setting youve obtained your Care Coordination experience in, youll be aware that no two days are the same, so youll already be able to evidence your adaptability and aptitude to work well under pressure, using your initiative and thinking on your feet.

Therefore, applicants must hold a Driving Licence and have access to a vehicle. You must also be committed to supporting the out of hours on call service, which is covered on a 1 weeknight per week and 1 weekend in 4 rotational basis across the team.

We're offering a competitive salary between £29,460 - £35,460 per annum, which includes payment for on-call.There is also the opportunity to earn bonus of up to £2,000pa.
