The Outpatient & Wellbeing Service delivers holistic multi professional non-pharmacological symptom management to those living with a life limiting illness. Focus is on supporting, encouraging and empowering people to work towards what is important to them alongside encouraging local community action.
Main duties of the job
The AHP Lead for Clinical Outreach will combine Professional practice and management skills in providing leadership and direction for the Outpatients and Wellbeing services and will include the following duties:
- You will lead and support the team in managing referrals from triage, monitor referral patterns and address any issues in a timely manner.
- You will provide direct clinical care, demonstrating specialist palliative practice in holistic assessment by planning, implementing and evaluating the physical, psychosocial and spiritual components of all patients and their carers and ensure the care is patient centred.
- You will provide the infrastructure including governance, to enable planned and unplanned clinic-based care to operate effectively and safely in a range of settings.
- You will lead the integration of Occupational Therapy and Physiotherapy services into Community Palliative Care East, enhancing outreach and accessibility for all service users
- You will work collaboratively with the Head of Community Palliative Care East, and senior Clinical Leaders to deliver clinics internally and within the community effectively i.e. symptom management interventions, Living Well partnership groups and peer support groups
- Work with the Head of Community Palliative Care East and professional/clinical leaders to deliver clinics, symptom management intervention and the Living Well partnership Groups within the community and onsite as well as managing Compassionate Neighbours.

Qualifications
Essential criteria- Degree/Master educated AHP
- Teaching qualification
- First Line management or Leadership qualification
Experience
Essential criteria- Extensive post registration experience
- Recent significant experience of working as Palliative care clinical practitioner
- Experience of leading and managing a team of clinicians-MDT
- Experience of working as a Specialist Palliative Care practitioner
- Specialist knowledge of palliative care and symptom management
- Awareness of the use of clinical supervision in practice
