Job overview

A fantastic opportunity has arisen for a Band 6 Lead Mental Health Nurse or a Band 5 ready to take that next step to being a Band 6 nurse , to join the older adult In-Reach and Home Treatment Team.

The primary goal of the service is to support people within their own homes, who are experiencing a breakdown of their mental health and to prevent admissions to hospital where appropriate.

Main duties of the job

The post holder will be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.

You will work collaboratively with colleagues, key partners and those within the local community to develop services to meet the mental health needs of the community.

The post holder will employ a wide range of skills, building on and developing their expertise and actively seeking advice and collaboration with issues which may not be readily within their sphere of competence.

As a senior registered practitioner you are professionally and legally accountable under the NMC code of professional practice.

We cover Derby City and the the surrounding areas of Derbyshire.

There is an expectation that as a Lead Nurse, you will be able to make complex decisions, lead a shift, cover duty and formulate detailed assessments and person-centred care plans to assist in the prevention of hospital admissions where appropriate.

Experience of working with older adults with a functional, mental health illness is essential.

The successful applicant will work within a supportive MDT and have close links with the Community Mental Health Teams, GPs and social care.

We are also co-located with our established, Dementia Rapid Response Team.

Professional development and further learning is encouraged and you will receive robust clinical and managerial supervision, along with wider support and training in-house within the team.

You will also be expected to provide supervision to team members and mentor students.

The team works a shift pattern covering, Monday-Friday, 9.00am-5.00pm; this includes Saturdays, Sundays and Bank Holidays on a rota basis.

The team covers 365 days a year, the ability to work shifts is essential.

  • Recent and relevant, post-qualification experience with functional older adults
Desirable criteria
  • Community experience

Qualifications

  • Nursing degree/diploma and NMC registration
  • Mentorship/SSSA trained
Desirable criteria
  • Supervision trained

Skills

Essential criteria
  • Knowledge of MHA/Capacity act/Legal frameworks
  • Able to meet the travel requirements of this post
  • Aware of own Limitations
  • Good clinical assessment skills
  • Good demonstration of risk assessment skills
  • Knowledge of functional illness
Desirable criteria
  • Evidence of effective risk management working
  • Good IT skills/ knowledge of computer systems.
