A new team is being established to work alongside the within the Unplanned Care Response Team for the West Place and to support insulin administration / diabetes care in the community. The applicant should ideally have excellent communication skills with a good understanding of IT systems.

Previous Community Nursing experience would be beneficial for this position.

We are looking for a well-motivated team player who will be expected to work in a busy environment and able to meet the demands of this hard-working service. Support and any additional training would be provided by the teams and Clinical Education Team to ensure competency and skills are established and maintained.

Key Areas of Responsibility
  1. To work within the integrated team to facilitate early discharge from hospital
  2. To work within the integrated team to prevent unnecessary admission to hospital
  3. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
  4. To work collaboratively with the Band 5 Peripatetic Insulin Nurse to ensure caseload is supported.
  5. To support West Unplanned Care Response Team with patient visits such as urgent INR’s, ecgs, wound care and double-up visits.
  6. To work closely with Triage and Diabetes Specialist Team to ensure new patient discharges are reviewed in the community setting.
  7. To support patients with re-enablement following Hospital Admission or new diagnosis, wherever suitable.
  8. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, insulin administration to our service users.

This is a new team that will be based within our expanding Triage team, we have already established a new Unplanned Care Response service and are looking to increase further to include the Peripatetic Insulin Team. We have a wealth of experience and knowledge and are led by an experienced Clinical Operations Manager.

We provide excellent evidence-based care to patients, ensuring that safe, relevant and timely care is provided by the community nursing teams to the local housebound population of West Norfolk. We work closely with our GP and Social Services colleagues and visit patients in their own homes, care homes and clinics as appropriate.

The post holder will:
  1. Apply competency-based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment.
  2. Plan and prioritise own visits to delegated patients and ensure these are documented in personal work diary.
  3. Ensure patient held records are completed for each visit and that SystmOne inputting is completed daily.
  4. Make basic judgements on patients’ response to prescribed treatment/care plans, and report findings to a qualified practitioner.
  5. Liaise with other healthcare professionals as required.
  6. Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.
  7. Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
  8. Demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, e.g. taking blood sugars, administering insulins, urgent INR’s, ecgs, wound care, supervising, transfers.
  9. Be responsible for ensuring equipment is used safely, following relevant training and that it is maintained appropriately.
  10. Contribute to the requisitioning of supplies and equipment through electronic ordering systems, and restocking of equipment.
  11. Work with qualified practitioners with group work/clinics as required.
  12. Demonstrate an awareness and understanding of consent and gain consent as appropriate following organisational policies.
  13. Share responsibility of indirect patient contact tasks such as answering telephones, arranging appointments, processing referrals and inputting activity data.
  14. Undertake health promotion / education as required, e.g. smoking cessation, falls etc.
  15. Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
