Location: Norwich Hours: Monday to Friday, 9 AM - 5 PM Rate: £19 per hour LTD £17.30 PAYE inclusive
Requirements:
HCPC Registration Experience in community assessments
PMVA required
What You'll Do: As part of our Older Adults Team, you will be responsible for conducting community-based assessments, working closely with individuals to support their health and well-being, and developing personalized care plans to promote independence in daily living.
