Job overview

Blaenau-Gwent are seeking an exceptional Highly Specialist Occupational Therapist to join our dynamic Integrated Community Resource Team. As part of our team, you will have the unique opportunity to help promote independence and empower individuals to achieve their personal goals.
You will be working closely with the Community Resource Team and the Information, Advice and Assistance (IAA) team, leading on the objectives of the "Better Care" and "Discharge to Recover & Assess" model. You will have strong links to the hospital therapy teams and the long-term social work teams as well as care providers.

You will be expected to lead on prevention and early intervention by adopting a positive risk taking initiative and by engaging individuals in a rights vs responsibilities conversation.

You will provide clinical and professional leadership to therapists within the Community Resource team, as well as being an integral part of the wider multidisciplinary service comprising of Occupational Therapists, Physiotherapists, Rapid Response Team, SALT, Community Ward's, Reablement and DASH/Home Care.

Main duties of the job

You will possess direct community experience and will be expected to carry a defined caseload of individuals requiring therapy support. You will be conversant with relevant legislation that informs the development of community care services locally and will be involved with any future proposed changes to services in Blaenau-Gwent.

The post holder will be responsible for planning and delivering the objectives of IAA alongside the Community Resource Team and the Long-term teams. They will be responsible for the co-ordination and evaluation of service delivery ensuring effective management of human and other resources.
The post holder will provide clinical and professional leadership to the therapists in the Community Resource Team and develop strong partnerships working with the front doors. The post holder will also lead on the objectives of the Better Care and D2RA model.

In addition to managing a varied caseload of patients they will have a line management responsibility for the therapists and will provide input to the planning, development and evaluation of the Community Resource Teams.

Fixed term until 31st March 2026 to meet the needs of the service
