Main duties of the job
Health & Justice Services are very excited to recruit to this role. The Healthcare Navigator role itself is new to our services and we have an opportunity to recruit a keen individual who has great initiative, transferable skills and exceptional Healthcare knowledge within prisons to promote and deliver care and guidance to our patients.
By adhering to the Trust values this role will provide care and support to those in the early days in custody with the aim to add significant value to their experience of the prisoner, prison staff in the initial days; reduce the risk of self-harm and suicide alongside wider impacts on families and friends.
This role will provide reassurance, guidance and signposting to health and social care services that will contribute to minimising distress and risk for those patients in their early days in custody.

Detailed job description and main responsibilities
MAIN DUTIES AND RESPONSIBILITIES- Offer a visible presence during their early days in custody and on the Induction Wing – therefore being accessible and available to all new receptions.
- Help to improve identification of risk factors during early days in custody.
- To be a bridge between HMPPS and the Healthcare providers to enable improvements in joint care.
- Address vulnerabilities associated with early days in custody inclusive of reducing self-harm.
- Improve Induction into establishments.
- Create greater awareness of healthcare and social care options and the range of services available.
- Support access to those services i.e. use of PODS , application process especially for people from diverse backgrounds or with specific needs
- Improve partnership working between services – particularly social care, Safer Custody teams.
- Improve engagement rates into healthcare and reduce did not attend appointments.
- Assist with improving patient choice regarding healthcare interventions.
- Further support social prescribing and promote and manage Health Champions.
- In conjunction with EPIC establish and run specific patient forums particularly relating to post ACCT.
- Input into complex case meetings.
- Be responsible for logging and making referrals
- To offer appropriate support and guidance to patients and their families/carers
- To plan / organise work using own initiative, whilst being able to work as a valuable member of a team.
- To have excellent IT skills, to include Microsoft Office, Outlook and Excel
- To undertake general office duties to support the role
- Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise and support where requested
Person specification
Education & Qualifications
Essential criteria- 5 GCSEs at grade C or above in Maths, English and Science
- A-level/NVQ level 3 or equivalent
- IT skills
- Foundation degree/level 1 diploma modules or willingness to undertake
- Health Promotion Qualifications/Certificates or willingness to undertake
Previous Experience
Essential criteria- Experience of working in a team
- Experience of Health and Safety practices
- Experience of working to deadlines
- Previous experience within a secure / prison environment
Skills & Knowledge
Essential criteria- Fluent written/spoken English
- Good organisation and prioritisation skills
- Good communication skills (written and oral)
- Basic word processing and spreadsheet skills
- Experience with using SystmOne (Clinical System)
Attitudes, aptitudes & personal characteristics
Essential criteria- Ability to motivate self and others
- Team worker able to relate to all grades of staff
- Able to work under pressure
