This is an exciting opportunity for practitioners to work as a Youth Support Worker within our Children and Young People crisis teams which includeHome Treatment, Rapid Response and Childrens Ward Assessments within the Pennine Care footprint.

We are looking for individuals who are passionate about supporting a team working with children and young people and working innovatively across pathways and traditional boundaries to ensure that young people get the right care, from the right clinician/team, at the right time and in the right place in order to improve their outcomes and the families/carers experience of mental health support.

This includes covering a variety of shifts over our 7 day service.

The post holder will have the opportunity to make profound improvements in how crisis care is experienced by children, young people and those who care for them. You will support the clinical members of the team to link with emergency services and primary care to offer first response assessment and intervention for children and young people in actual or emerging crisis.

You will offer intensive follow up for up to 6 weeks and will support the development of longer term care plans to identify and address the holistic needs of young people and families. You will also maintain links with young people admitted to specialist in-patient services, to ensure young people retain links with their communities, within a recovery focused model.
