Phlebotomy Using technical equipment to safely draw blood from patients Explaining the process of drawing blood clearly to a patient Answering patient questions regarding collection techniques and offering additional resources on the collection process, if the patient requests them Reassuring and calming patients who might be nervous or scared of the procedure Ensuring that the patient is as comfortable and relaxed as possible and confirming their agreement to the collection of a blood sample Checking patient identification against GP/Nurse request and confirming with patient whenever possible Accurate labelling of samples to confirm patient identity Applying dressings to the wound created by drawing blood Sending blood samples to testing laboratories Ensure that domiciliary blood samples are returned to the hub in time for courier collection and on Saturdays, ensure that domiciliary blood samples are taken to the lab at the Horton Hospital at the end of the shift.

Frailty Healthcare Identify frail vulnerable groups of patients in the community to assess whether there are any unmet health or care needs. Undertake basic assessments such as taking weight, BP, foot checks and pulse rates to ensure that any housebound patient you are seeing is receiving sufficient care and to contribute to the management of their long-term conditions, eliminating the need for them to visit our practices for support.
Perform baseline assessments when appropriate to detect early signs of deterioration in health for housebound patients. When necessary identify urgent needs, taking relevant action or liaison Notifying the patients GP if the assessment gives rise to any concern Refer and liaise with a range of statutory and voluntary sector support agencies, ensuring the person has access to a wide range of support Provide guidance and support to increase the use of self-care methods.
General Keep timely, accurate and complete electronic patient records and activity data on EMIS, all of which will be maintained and stored in accordance with professional and organisational standards. To have responsibility for appropriate documentation related to patients in their care to be disseminated to all relevant professionals in a timely manner and in accordance with operating standards.

Notifying GP/Nurse of any significant patient issues During holiday periods you may be required to assist the phlebotomy teams at any of the NORA practices. Be responsible for maintaining Health and Safety standards within the business, in line with the Health and Safety policy To ensure ongoing compliance with CQC essential standards To ensure compliance with all GDPR and data security policies Adhere to all business protocols and SOPs To comply with the Practice HR policies Ensure effective and professional communications at all times Ensure a high standard of personal presentation at all times To take an active part in the performance review system Complete relevant (mandatory) training and ensure agreed deadlines are met Attend internal and external meetings as required To support sustainability practices and recognise the shared responsibility of carrying out duties in a resource-efficient way This job description sets out the main duties of the post at the date it was written.

Duties may vary without changing the general character of the post or the level of responsibility entailed. As a Frailty/ Phlebotomist Care Coordinator, you will be required to work to an appointment list for patients in their own homes based across the NORA network area.

To do this role a clean UK driving licence and your own vehicle is essential.
