Description
We are seeking a dedicated and compassionate Community Care Officer Level 2 - Adults to join our dynamic team. This vital role involves working closely with adults in the community, providing them with the support and care they need to live fulfilling and independent lives.The Community Care Officer will be responsible for assessing client needs, creating personalized care plans, and ensuring that each individual receives high-quality assistance tailored to their unique circumstances. You will play a crucial role in promoting the wellbeing and dignity of our clients, empowering them to maintain their independence while also providing them with necessary support.
You will collaborate with a multidisciplinary team, local agencies, and families to deliver seamless care and services. In addition, you will monitor client progress, adapt care plans as needed, and ensure that all interventions comply with current regulations and best practices in community care.

ResponsibilitiesAssessing the needs of adults within the community and developing personalized care plans.
Monitoring the wellbeing of clients and providing ongoing support to meet their needs.
Collaborating with other healthcare professionals and community organizations to coordinate care.
Conducting regular reviews of care plans and adjusting as needed based on client feedback and changing circumstances.
Providing emotional support and practical assistance to clients and their families.
Maintaining accurate records of client interactions and progress in line with compliance requirements.
Staying informed about local resources and services to assist clients effectively.
RequirementsRelevant qualifications in health or social care, such as a Level 2 diploma in health and social care.
Experience working with adults in a community care setting or similar environment.
Strong communication and interpersonal skills to build rapport with clients and their families.
Empathy and compassion for individuals facing various challenges and vulnerabilities.
Ability to work both independently and as part of a team in a fast-paced environment.
Knowledge of safeguarding policies and procedures related to adult care.
Excellent organizational skills to manage multiple cases and priorities effectively.
Hours Per Week: 37.00
Start Time: 09:00End Time: 17:00
Pay Per Hour: £13.47
Location: Cardiff, Cardiff County
