Cancer Support Worker | University Hospitals Bristol and Weston NHS Foundation Trust
We are looking for an enthusiastic and motivated person to join our compassionate and highly skilled team of Cancer Support Workers, to provide support to people diagnosed with cancer.
A Cancer Support Worker works as part of the cancer care team, alongside registered practitioners to help improve and provide a personalised care and support package for patients and their families, supporting and signposting with the aim to facilitate a self-managed approach and a firm understanding of their cancer pathway.
As a Cancer Support Worker you can offer practical and emotional support, signpost and provide information around lifestyle changes such as diet and physical activity and assist with practical, social & emotional concerns. This support is delivered by carrying out telephone support to patients, undertaking holistic needs assessments and care planning and organising and providing personalised care and support activities for people living with and beyond cancer.
You will work in the Cancer Information and Support Centre on a rota basis, along with supporting health and wellbeing events for people affected by cancer, both virtually and face-to-face in the future.
Please note, this is not a health care assistant role.- Triage incoming calls and initiate appropriate response according to protocols and individual pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non routine and refer complex decisions to the clinical team for assessment and review
- Provide basic telephone advice and refer on or sign-post to other sources of support
- Coordinate the necessary assessments, appointments or investigations to fast track people back into the Service
- Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or an emergency situation, alerting the clinical team or enabling rapid response as appropriate
