Responsible for day-to-day co-ordination of operational tasks and staff (as applicable), allocation and completion of work. Management of aligned administrators, including supervision of daily activities, task allocation, monthly supervision and contribution to annual appraisals.
DELIVERING RESULTS/FUNCTIONAL RESPONSIBILITIES To provide a comprehensive administration service, give support and advice to the AACC team. Tasks may range across AACC processes; co-ordination of initial assessments, verification/outcomes, care planning, brokerage, care reviews, including project work, quality assurance/data quality, internal job rotation and absence cover.
To process work in accordance with internal Standard Operating Procedures and ensure that clinical and financial related documentation is entered onto databases for all applicants/clients in a timely manner. To liaise with families or representatives, ensuring they are kept fully informed and up to date throughout the AACC process.
To receive sensitive information via patient notes and reports from GPs, hospitals, care homes, Social Services, district nurses and other health professionals along with submissions from individuals and their representatives. To request, monitor and respond to stakeholders to request for information, communicating and escalating to line managers where necessary to meet strict timeframes.
To be responsible for photocopying and checking legal documentation e.g., Power of Attorney and identification documents and returning them to sender. Compile and maintain electronic individual files for all cases and source additional information as required to enable the CHC process to progress.
To understand the principles of electronic file management, apply them to daily work and adhere to the records management policies. Ensure that patient confidentiality and the sharing of records is according to relevant legislation e.g., Data Protection Legislation, Information Governance and Caldicott Guidance.
Liaise with senior staff to ensure cases are allocated to professional staff to enable their completion. To liaise with patient/representatives and stakeholders to arrange appointments for Continuing Healthcare assessments and care reviews. To undertake general office duties including filing, processing incoming and outgoing post as well as scanning and photocopying.
To help support the management of the team email boxes. Monitor progress of cases and liaise with senior staff to ensure agreed timescales are adhered to. To type confidential correspondence ensuring all decisions in respect of Continuing Healthcare are conveyed to all relevant parties in agreed timescales adhering to the National Framework and local policies.
Ensure that all outgoing and internal material reflects a high standard according to the ICB and NHSs identity principles. Ensuring information is accessible where there are barriers to understanding. To respond to Subject Access Request (SARs) in a timely and appropriate manner.

Produce reports as required on activity and performance. To develop the role and undertake on-going training courses to meet the needs of the service and policy changes. Develop a good working knowledge of NHS Continuing Healthcare and NHS-funded Nursing Care (FNC), the supporting National Framework and its relationship with Social Services funding.
