As a Nurse Assessor you will be using your clinical skills and knowledge to assess patients using the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care. You should have knowledge of a variety of specialist areas acquired through experience and training, plus further specialised knowledge of the personalised healthcare commissioning process.(Continuing Healthcare). Knowledge and experience of working with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care is essential.
Main duties of the job
You would be expected to lead or participate in multidisciplinary meetings either Face to Face or remotely via Microsoft TEAMS , and be able to communicate complex issues to a wide range of health and social care professionals, patients, carers and family members.Therefore, excellent communications skills, both verbal and written, are essential.
You will be required to maintain contemporaneous electronic records and complete assessments and reviews on-line on the data system therefore IT skills are essential to the role.
You would need to be an experienced negotiator, pro-active and have a positive approach to problem solving. The ability to analyse complex and often conflicting information from various sources, to aid decision making, is imperative, as is the ability to work under pressure, to meet tight and frequently changing deadlines.
Our work is multi faceted and we work alongside other professionals to commission packages of care or support with care home placements for a variety of clients from a wide demographic background.
Our nurses also provide on a rota, duty cover to ensure referrals into the service are appropriately triaged and or signposted to other relevant services and disciplines as required.

Our nurses also assess client's for eligibility for Continuing Healthcare funding.As a CHC nurse practitioner this would include case management in line with the national framework and management of CHC packages of care, ensuring value for money placements which meet the needs of the individual and identify opportunities for development and improvement to assist delivery of best practice healthcare.
The post holder will ensure that they, and the team, always adhere to the NHS Continuing Healthcare National Framework, and promote the use of Personal Health Budgets(PHB's); working within established policies, procedures, protocols and systems, ensuring quality standards and performance.
You will: Foster transparency and a person-centred approach with patients, families and representatives.
Promote the use of PHB's for eligible individuals as a means of having control over their care and achieving best possible outcomes whilst being cost effective and value for money.
Ensure the principles and duties of safeguarding are holistically and consistently applied
Assist the Clinical & Operational Manager and Clinical Team Leader for CHC with resolution of complaints, appeals and disputes.
Work within a team of CHC Clinical Assessors, taking a lead role in Continuing Healthcare cases, ensuring a high-quality service is delivered, supporting and mentoring staff to achieve continuous improvement.
Facilitate and contribute to multi-disciplinary assessments and patient reviews in line with standard operating procedures.

Healthcare Professional Registration with the NMC; RN and educated to degree level in relevant subject or equivalent level of qualification or experience in working at a similar level in specialist area.
Minimum of 5 years post registration experience with evidence of working within a Multidisciplinary team-
Excellent nursing assessment skills and the ability to communicate effectively using oral and written communication
Excellent organisational skills with the ability to self-manage workload and meet deadlines
Well motivated and willingness to work autonomously
This post involves travelling to visit individuals to complete face to face assessments so car driver and use of own vehicle is essential

Essential
Essential criteria- Registered General Nurse with current NMC UK registration, with 5 years post registration experience in a relevant clinical setting
- Knowledge of Continuing Healthcare and the NHS National Framework for Continuing Healthcare
- Experience of conducting and participating in Multi Disciplinary Team meetings and liaising with health and social care professionals.
- Competence in using clinical systems, emails, Microsoft Office applications and electronic patient record systems, strong written and verbal communication skills
- Car driver with access to a vehicle on a daily basis with full UK driving license
