Rehabilitation Assistant Function: Hospitals and Rehabilitation Department: Rehabilitation Therapies Reporting Manager: Clinical Services Manager About this role: As a Rehabilitation Assistant you will work as part of the wider multidisciplinary team to maintain and develop high standards of rehabilitation and care within our inpatient and outpatient services.
You will have the opportunity to work alongside our highly specialist team of Neurological and Musculoskeletal Physiotherapists, Occupational Therapists, Speech and Language therapists, Dietitians, Neuro Psychologist and the medical team to provide individual needs, therapeutic programmes and care plans out lined by the treating therapist.
You will be involved in the treatment of a wide variety of Neurological and musculoskeletal conditions following illness, injury and or trauma. You will be providing practical assistance and encouragement to develop patient/client centred goals, to preform everyday living skills, tasks and leisure activities with a focus on empowering and supporting patients/clients to realise their potential to live a fulfilled life.

Empathetic and have a desire to make a difference Passionate about forming therapeutic relationships Committed to a person-centred approach to recovery Enthusiastic about working as part of a dedicated team Resilient and able to work under pressure An effective communicator with good written and verbal skills Willing to take ownership for the delivery of quality care and demonstrate leadership qualities To be a great motivator Core Responsibilities and Key Accountabilities: Work proactively with the nursing and therapy teams to implement individualised rehabilitation programmes, following plans of care to meet patient /service user centred outcomes.

Undertake observations such as temperature, pulse, blood pressure recordings and monitoring and recording of clients weight when indicated. Follow the highest standards of care, and intervention sensitively and respectfully. Encourage and enable patients and service users to enjoy meals and drinks while supporting wishes and preferences.

This may include assisting with ordering of meals, monitoring and recording fluid intake and assisting with feeding. Assist with communication, ensuring that patients and service users can be understood and understand others. Assisting with mobility including hoisting, positioning and assisting with mobility aids To work with therapists/team members to ensure optimum cognitive, social, emotional, physical and functional recovery and to monitor the physical and emotional well-being of all clients in order to be aware of changes and to alert senior colleagues as necessary.

Support patients and service users to access the community safely including with activities and appointments. Support patients and service user to achieve a purposeful life, contributing to occupational, educational, social and leisure activities and assisting in developing programmes to meet those needs.
Gain an understanding of personal, emotional and social needs of patients and service users including the behavioural and emotional changes which can be associated with neurology and or trauma. Participate in the keyworker programme, acting as an advocate and liaising with others regarding individual programmes/plans to ensure a consistent delivery of care and rehabilitation.
Develop and engage in therapeutic relationships with patients and service users whilst maintaining professional boundaries. Complete written records in line with legal frameworks. To take responsibility for own personal and professional development through attendance and engagement in supervision and appraisal.
Contribute to service development by engaging in staff forums and feedback groups. Have the ability to work in a dynamic and changing environment. To be involved in clinical audits as and when directed. To contribute to the preparation, maintenance, and general housekeeping of equipment and environment.
