To be the partnership manager for the locality and respond to necessary internal and external requests relating to the area. To work with colleagues across the ICB to ensure that there is effective collaborative working on strategy development and service redesign with key partners, including patient & public representative groups.
o To be the Primary Care Partnership lead, have expert up to date, evidence-based knowledge across various disciplines and functions and sound knowledge of associated and related areas. To understand and utilise the tools and techniques of change management within and across organisations and professions.
To plan, negotiate and agree allocation of the necessary resources to ensure projects can be delivered. To manage allocated financial resources. To support the development of strategies to transform health and social care for the region including: reducing health inequalities, increasing preventative care and the availability of services in the most appropriate setting.
To work with the PCN Clinical Directors, Executives and Heads of departments to develop strategies. To be responsible for the delivery of the Primary Care Strategy as it relates to practices within the locality. To be responsible for supporting PCN development in the locality.
To take the lead on specific areas of the Primary Care Strategy and to assist with future strategic developments. To lead on specific project areas across the wider localities where the need is across the county as a place. To support the development of ICB corporate plans (QIPP, operational and commissioning intentions) for the relevant area of responsibility.
To be responsible for the implementation of agreed plans, monitoring progress, identifying and taking appropriate action to ensure the programme progresses. To establish and maintain project monitoring and control mechanisms which enable projects to be successfully delivered in a work environment that is subject to frequent change.
To design and audit projects to inform progress/success of the programmes. To utilise the Commissioning Support Unit's informatics services throughout all redesign activities, ensuring best use is made of available commissioning knowledge and intelligence across the health economy.
To lead the development and implementation of communication strategies to support the work of the programme, including where the information is complex/sensitive and there may be resistance. To communicate and provide regular reports to organisations executive teams on the progress and recommendations from the programme.
To support opportunities for change and innovation in current providers in response to community/patient feedback and changing commissioning requirements. To produce clear visual material and data for use in reports, briefings, training and meetings for internal and external presentation.

To draft reports to provide response/feedback from the ICB on national documents to NHS England and other stakeholders.
