Main duties of the job
  1. To provide a high quality ward based clinical pharmacy service, rotating through the various rotations This includes reviewing drug charts for safety and efficiency, drug history taking and medicines reconciliation, medicine supply, patient medication consultations, medicines optimisation services, discharge medication service (DMS) and any other activities associated with medicines management.
  2. To participate in departmental and regional education and training programmes. In particular to undertake study towards obtaining the Certificate/Diploma in General Pharmacy Practice.
  3. To provide pharmaceutical advice on both supply and clinical issues to other healthcare professionals as requested.
  4. To provide medication related advice to patients, focussing on medicines stopped/started or changed in hospital and side effects profiles enabling them to obtain the maximum benefit from their medicines.
  5. To assist in the education and training of pre-registration pharmacists and other staff in training grades.
  6. To supervise the daily activities of junior pharmacy technicians and pharmacy assistants.
  7. To participate in all activities of the pharmacy dispensary as required, including dispensing, clinical verification of prescriptions, prescription review, accuracy checking of prescriptions and a high quality patient medication consultation service.
  8. To participate in late nights, weekend, bank holiday shifts

Detailed job description and main responsibilities

For pharmacists recruited at the band 6 entry point a pathway identifies criteria for progression and highlights the gradual introduction of key responsibilities as experience is gained working as a Band 6 Pharmacist. After typically being in post 19-24 months and being able to demonstrate they have met the key responsibilities the pharmacist will be eligible for progression to a band 7 post.

Qualifications/Training

Essential criteria
  • Pharmacy degree (Masters level)
  • GPhC Registration as a Pharmacist or Provisionally Registered Pharmacist.
  • Willing to undertake clinical training to post grad diploma level
Desirable criteria
  • Train the Trainer qualification
  • Membership of the Royal Pharmaceutical Society (RPS)
  • RPS Faculty Member portfolio in preparation

Experience
