Role Summary To support the Diabetes Specialist Team in providing patient education. To ensure patients are receiving appropriate health screening and checks related to their diabetes management and promptly communicate any concerns to the Diabetes team.
To be a contact for patients receiving care from SDHC Specialist Community Diabetes service. Principle Duties to include: COMMUNICATION AND ENGAGEMENT To provide support to the patients to help them make informed decisions about their care. This support can be offered through face-to-face, telephone discussions, or by responding to email queries.
To provide individualised diabetes care tailored to patient needs and circumstances. To provide diabetes education to patients, covering topics such as hypoglycaemia management, insulin initiation teaching and to completion of the Gold score questionnaire.
To ensure smooth running of the clinics by supporting clinicians in various duties, including blood pressure monitoring, weight measurement and blood glucose or ketone monitoring. To support patients to download their glucometer data onto Diasend before their appointment time.
To liaise with patients regarding annual health checks to improve patient safety To support diabetes team with health promotion and awareness events such as Insulin Safety Week, Hypo Awareness Week and World Diabetes Day. This will includes sourcing promotional materials, organising venues and stands, advertising the events and engaging with public.
To provide education to patients on how to use blood glucose and ketone meter. To ensure education materials are accessible to patients. This may require liaising with company representatives for leaflets, study materials and demo kits. To support the diabetes team in delivering Freestyle Libre and Bertie educational sessions.
To ensure diabetes stocks are up to date and to order any items that have run out from representatives. To maintain folders for use in diabetes clinics containing relevant educational materials. To establish and maintain effective communication with diabetes team and patients.
To maintain a professional attitude and appropriate appearance at all times in accordance with Trust Dress Code Policies. To improve patient experience and satisfaction in diabetes management. Foster effective communication channels between the diabetes team and patients, addressing inquiries and concerns promptly and professionally.
Possess a valid driving license and the ability to safely drive if required for travel between Trust sites or other job-related duties. EDUCATION AND DEVELOPMENT Use available opportunities to maintain and extend knowledge, ensuring competence in the required skills.
To support the Diabetes team in conducting clinical audits as required Ensure compliance with Trust mandatory training To participate in the Trusts appraisal system with Line Manager and attend training programmes and clinical supervision as identified within a Personal Development Plan.
CLINICAL GOVERNANCE To support the diabetes team in conducting clinical audits and implementing changes to meet patient requirements and contribute to service development. To ensure rigorous adherence to infection control safety practices, including hand hygiene and universal precautions.
To ensure timely, accurate, and comprehensive record-keeping, complying with Trust standards and guidelines. To understand and be aware of when and how to report incidents or near misses. To ensure the implementation of all policies, procedures, and quality standards as required by the Trust.

To protect and maintain patient confidentiality at all times. To respect the individual values, cultural, and religious diversity of patients, contributing to the provision of services sensitive to these needs. To support the service and organization by performing any other duties for which qualified and able.
