We are excited to announce that we have a vacancy in the following post –

Deputy Locality Manager – Welwyn and Hatfield Integrated Care Team – 37.5 hours per week.

The post holder will be expected to use a range of reports and approaches to identify key areas for improvement within the team and to work closely with community matrons, the locality clinical lead, the therapy lead and the locality manager to implement change successfully.

The role of the Deputy Locality Manager is a pivotal one in the locality team, to be able to deliver the Integrated Care Team in line with the service specification to deliver the key performance and quality indicators.

Whilst the role is predominantly non-clinical the post holder is expected to maintain their clinical competence and act as a clinical role model.

We are looking for someone who is passionate about contributing to the success of the service.

In addition to the basic salary, this post attracts a High-Cost Area Supplement.

A full valid driving license and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Responsible for the day to day management of both the team and caseload, ensuring adequate staffing levels are maintained and a high standard of care is achieved meeting patient expectations.

Ensure team/staff provide safe clinical care in patients’ homes through agreed pathways concentrating on the demanding/complex cases that may require advanced assessment and management skills.

Provide on-going support to those patients, families and carers with complex needs, where there may be emotional, physical and life threatening conditions. Provide support to other team members in relation to this work.

Ensure the maintenance of adequate staffing levels, utilising e-rostering software as appropriate, meeting the required timelines for roster sign off. Risk report and escalate to line manager where required resource cannot be achieved.

Operational Delivery

Responsible for the day to day management of both the team and caseload, ensuring adequate staffing levels are maintained and a high standard of care is achieved meeting patient expectations.

Ensure team/staff provide safe clinical care in patients’ homes through agreed pathways concentrating on the demanding/complex cases that may require advanced assessment and management skills.

Supporting the team working within variable conditions in patient’s homes and delivering care in health centres, GP surgeries and community venues as required to manage and guide delivery of quality service by staff.

To support the health and wellbeing of the team including the achievement of required vaccination targets e.g. flu. Acting as a role mode and ensuring that all staff are informed and compliant unless there are substantial reasons why this is not possible.

Provide support to locality manager, keeping them informed regularly of team developments and significant issues. Meet on a planned basis with locality manager and other team managers.

Advise on clinical procedures within maintained scope of competency, utilising a variety of clinical instruments and equipment safely and precisely.

Strategic Management

Planning and organising team performance.

Ensure the maintenance of adequate staffing levels, utilising e-rostering software as appropriate, meeting the required timelines for roster sign off. Risk report and escalate to line manager where required resource cannot be achieved.

To manage the annual leave process, ensuring that the locality has processes in place to ensure resilience across the year.

Identify the health needs of the practice population and establish priorities, targeting vulnerable groups, in line with local and national initiatives.

Be prepared to participate in the HCT Emergency Plan in response to major incidents as requested. Carry continuing responsibility for ensuring that the multi professional team deliver effective assessment of physical, emotional and social health care needs and develop implement and evaluate programmes of care effectively.

Regularly assess and utilise data reports and performance information to monitor performance and drive improvements including late data entry.

Service Development and Improvement

To act as a resource within HCT concerning developments within community services, taking

forward initiatives and sharing best practice.

Ensure that team members have a clear understanding of the relevant HCT policies and implement them as required.

Deal with Complaints and Clinical Incidents, in line with Trust policies, completing necessary paperwork and informing appropriate officers within the time frames stated in the policy, de-brief staff involved and implement changes to practice in line with findings.
