Job overview

The post holder will be an autonomous practitioner, working within codes of practice and will manage a varied and complex caseload providing
highly skilled Occupational Therapy intervention for Monmouthshire residents who are in their own home and in-patients at Monnow Vale
Health and Social Care Facility Monmouth. To facilitate the maintenance and improvement in their wellbeing to complete their activities of
daily living, through Reablement and any other OT Intervention that may be required. Peers or external reference points may provide

guidance

  • To assess and address occupational therapy performance skills in the areas of mobility, sensory deficits, intrapersonal skills, intrapersonal
areas and cognitive skills, self-care, productivity and leisure.
  • To assess and interpret patient conditions and provide occupational therapy interventions in cases presenting a high degree of complexity
in an autonomous manner.
  • Communicates condition related information, sensitive information to patients and families which requires empathy & reassurance.
Supporting them to take responsibility for decision making and goal setting.
  • To identify and order equipment needed for patients to maintain their independence within their home environment.
  • To maintain records and documentation required by work settings in accordance with the Monmouthshire Integrated Service and COT
standards.
  • To attend and participate in wards rounds/case conferences/MDT’s.
  • To adhere to the COT code of professional conduct and ethics including confidentiality.

Detailed job description and main responsibilities

To demonstrate specialist knowledge relevant to the application of occupational therapy in one or more clinical areas.
  • To carry out specialist occupational therapy techniques.
  • To carry out specialist manual handling assessments and provide plans for others to follow in accordance to health and safety practice
guidance, to help maintain the well-being of individuals and those assisting with manual handling tasks.
  • To analyse, evaluate, interpret and communicate an overview of the patterns of referral, interventions and outcomes of intervention.
  • To take personal responsibility for own workload within a specialist area of practice.
  • To draw on evidence to justify own practice.
  • To be proficient in reasoning and in determining intervention strategies in complex situations and individual’s with complex needs.
  • To lead and be proactive in developing clinical networks as a shared governance activity.

Qualifications

Essential criteria
  • Degree or postgraduate Diploma in Occupational Therapy
  • HCPC Registration
Desirable criteria
  • Specialist knowledge and application of OT assessments and interventions relevant to client group
  • Practice Placement Educator

Experience

  • Evidence of CPD Commitment to lifelong learning
Desirable criteria
  • Membership of BAOT Membership of relevant special interest groups Apple accreditation
