Main duties of the job

District and Community nurses visit people who are housebound of all ages 18+ in their own homes or residential homes, enabling them to self-manage where possible and implementing appropriate treatments and care pathways. We are looking for a talented district nurse with a specialist practitioner qualification to join our innovative District Nursing Service.

Our service operates on a 24 hour basis we have opportunities in our day and evening teams as well as flexible and part time working to fit in with your work/life balance.

You will be an excellent communicator and team player with the drive and determination in case management and leading the delivery of high quality nursing care for your team. The successful candidate will also have the exciting opportunity to lead, facilitate and participate in the development of transformational programmes that support the development of new ways of working and new pathways within the integrated care organisation

Detailed job description and main responsibilities

To have overall continuing responsibility for the effective and efficient delivery of nursing care to patients within the District Nursing service and in a manner that at all times reflects the NMC Code of Conduct.

To take an active part in the implementation and delivery of the district nursing service and Whittington Health emergency plans as required by the lead district nurse/ service manager.

To contribute to the development of the nursing team and the community health services by participating in working parties, committees, planning and discussion groups.

To participate in clinical audit in line with clinical governance leadership.

To keep staff records (both paper and electronic) up to date and perform regular appraisals, one to one meetings and develop staff professional development plans.
To ensure that all staff are undertaking clinical supervision and to monitor this at monthly 1;1 meetings.

To teach and supervise the clinical practice of team members and to appraise and to identify training needs of all directly managed staff.

Education / Qualifications

Essential criteria
  • Registered Nurse ( RGN 1ST Level Registration )
  • District Nursing specialist practitioner qualification
  • Community Practitioner Prescriber
  • Independent nurse prescriber or willingness to undertake course
  • Advanced physical assessment course or willingness to undertake
  • Evidence of recent training and development
  • Up to date PREP portfolio
