Clinical CareGovernance andQuality Work in a Strengths Based way, maximising independence, and promotingwellbeing. Assess complex mental health and social care needs of serviceusers and carers, and the provision in place aimed at meeting this need.Assess and plan in line with the requirements of the Care Act (2014), s.117of Mental Health Act 1983 (as amended by the Mental Health Act 2007),Mental Capacity Act 2005.Apply theoretical knowledge and understanding of psychology, sociology,psychiatry and physical science to develop a case formulation describing anindividuals strengths and challenges.Produce a support plan for providers which meets the desired outcomes ofthe individual but also maximises independence.

Commission/implementthe right level of package of care at the right time so that independence ispromoted and resources maximised.Prepare applications to funding panel following review.Produce court reports for the Court of Protection.Convene and attend meetings in accordance with s117 and CPArequirements ensuring health and social care aftercare provisions andlegislative requirements under s117 are met.If qualified AMHP- Act as an Approved Mental Health Professional (AMHP),balancing the human rights of the service user within a legislative andstatutory framework alongside the issues of risk and safety to achieve theleast restrictive alternative.Where applicable make referrals to Client Financial Services.Actively access and evaluate ongoing up to date research to underpin,develop and inform clinical practice.Nurse Applicants-Confidently complete Nursing Needs assessments, CHCscreens and attend CHC MDTs.

Utilise side effect monitoring tools egGASS and Lunsers Develop an expertise in the bio psycho social assessment ofindependent living ability and be able to select and use assessmentsto illuminate underlying issues affecting independent living such asskills, routines or volitionAdvise the MDT and other agencies on the factors impacting onindependent living ability of the service user and support needed.Make recommendation to improve the residential environment of theservice user in relation to occupational participation and theachievement of their personal goals.From assessment and formulation, advise the MDT, funding panel,housing and care agencies, local authority and court of protectionregarding the ability of the service user to live safely andindependently and the support needed to do so.

Financial and Otherresource Exercise good personal time management, punctuality and consistentreliable attendance.Maximise access to resources across the provider market and utiliseuniversal services wherever possible.Ensure mechanisms in place to enable Fairer Charging.

Learning, Educationand PersonalDevelopment. Participate in regular formal managerial and professional/clinical supervisionand a shared appraisal of work performance with the line manager andparticipate in regular professional supervision Engage in continued professional development relevant to role and supportthe continued development of others through training and supervision.Contribute to the delivery of multi-disciplinary training and induction of newstaff.Act a Practice Educator for students within sphere of professionalqualification.Supervise team support staff and develop and oversee support plans whichthey will carry out aimed at promoting independenceAttend Professional ForumsMaintain Professional Registration Safeguarding Childrenand Vulnerable Adults Adhere to the principles of the Care Act in relation to the safeguarding ofadults with Care and Support needs.

Raising referrals and contributing tomulti agency safeguarding planning meetings. Work in an empowering andproportionate way. Contribute to the development of an agreed protectionplan enabling desired outcomes to be achieved.Work in line with Manchester Safeguarding Partnership and GMMHsafeguarding policies.Identify safeguarding concerns for children and young people and liaise withChildrens Social Care, Early Help to ensure information is shared andappropriate action taken.
