Provide education, advice and support to inpatients with diabetes and their carers, under the supervision of the Diabetes Specialist Nurses.
  • The role of this will include supporting patients in a variety of clinics such as pregnancy and diabetes clinics and new patient clinics.
  • Work collaboratively with multidisciplinary team to promote integrated care for inpatients within the diabetes care.
  • To provide a high standard of diabetes care. To act in a consultative role to patients, relatives, and health professionals on the management of diabetes mellitus
  • The post holder will utilise skills to assess and identify inpatient needs and implement delivery of appropriate individualised care for people with diabetes.
  • The post holder will take daily responsibility for managing a defined caseload, with the support of existing nursing and medical staff. Specialist holistic advice and education will be provided for in patients and their carers, referring on as appropriate for follow up care on discharge.
  • As a member of the diabetes nursing team, maintains relevant statistics to monitor inpatient activity and workload.
  • Communicates effectively with hospital and community services relating to:
  1. The hospital to home transition period
  2. Follow up and stabilisation of diabetes in the community
  • Works with other members of the multi-disciplinary team to provide a comprehensive care package to patients with diabetes.
  • Assessment of inpatients with diabetes to help identify physical and emotional problems relating to the condition
  • Educate and support inpatients, relatives, and carers to facilitate optimum diabetes control.
  • Able to refer to the appropriate member of the multidisciplinary team to assist with problem solving
  • Facilitates continuity of care and communication.
  • Utilises reflective practice to improve care.
  • Able to access up to date information on diabetes
